Integrated Care Pilots

UK’s first Integrated Care pilot programme to improve patient care was announced today by NHS Surrey with Integrated Health Partners.

Integrated Health Partners (IHP) and NHS Surrey today announced a local ICO pilot, which began in January 2009.  The pilot will improve patient care by supporting different health and social care professionals to work more closely together.

The pilot group, named the Guildford IHP partnership, covers approx 73,500 patients, and is a collaboration of six GP practices in and around Guildford together with IHP.  Although this pilot is launched outside of the Department of Health-led national pilots, it is consistent with the DH vision and demonstrates to PCTs and GPs that local pilots are possible.

The pilot, which is expected to achieve a demonstrable improvement in outcomes across a number of dimensions, will test and refine “team-based” care which patients will experience as “one service”.  This will be achieved by GPs and other health and social care professionals from different organisations, but involved in the care of the same patient, working more closely together as one team. This will simplify care for patients with long-term conditions who typically have to deal with care providers across a number of organisations.

Targeted changes include:

• Better meeting patient needs by greater coordination of GP, hospital, social and community-based care
• Improved access to services
• Greater involvement of patients in decisions on their care 
• More support and education for patients and carers to better manage conditions

To drive these changes GPs will be given more accountability for healthcare budgets allowing them to provide better choices for their patients. The GP practices involved are Dapdune House, The Guildowns Group, Shere Surgery & Dispensary, St Lukes, Wonersh and Woodbridge Hill.
