As part of modernising the pathway for patients referred to secondary care with haematuria, South Leeds PCT commissioned a primary care based cystoscopy service. Hospital referrals for patients with haematuria are triaged by a GPwSI and those who fit the appropriate criteria are directed to the community based cystoscopy service as part of the diagnostic process.
This is delivered from a local practice by GPwSI’s trained in the procedure with clinical support from a hospital-based consultant. Following results patients are either discharged, referred back to their GP or referred on to secondary care. The introduction of this service has reduced the waiting time for patients requiring cystoscopy, reduced the demand on the local acute Trust and avoided the requirement for many patients to travel to hospital.
In this current financial year the cytoscopy service has treated 87 patients tests across the five Leeds PCTs.
Key lessons learnt: Successful pathway redesign requires reassessment and adjustment following implementation
Barriers: Identification of appropriate primary care premises for relatively complicated procedures Length of time to train GPwSIs
