PBC:Toolkit Towards World Class Commissioning

Aim of Module

  • To be used as a teaching/learning aid for use in a number of settings
  • Practices, consortia and Practice Based Commissioning (PBC) groups
  • Engaging other healthcare professionals – nurses / dieticians / physiotherapists etc
  • Engaging other independent contractors – pharmacist / dentists / optometrists
  • Local authority / social services and others wishing to understand PBC
  • For others both within and outside the NHS for training

Module 1

What is Practice Based Commissioning and the Commissioning Cycle?

Contents

  • Definition of commissioning – ‘Best use of llocated resources to achieve optimal health outcomes’
  • How commissioning is developing – ‘World Class Commissioning’
  • How does PBC fit into this?
  • Outline of policy/guidance released to date about PBC
  • Link to goals and priorities of the localhealth economy
  • PBC and Primary Care Trust (PCT)priority planning
  • PBC consortia set up – rules of engagement / accountability agreements / Governance / support arrangements

Focus on the following goals:

  • Improving health and well being – ‘adding life to years and years to life’
  • Reducing health inequalities and social exclusion
  • Improving access to health services
  • Delivering quality, effectiveness and efficiency of services
  • Actively working to the Choice Agenda

The Toolkit will contain:

  • Presentations
  • Literature
  • Examples of good practice

Aim of Module

This module is designed to support the PBC consortia to operate effectively with the PCT and support the operational processes needed for commissioning and service redesign.

To achieve this aim the module will look in more detail at how the PBC practice or group and the PCT co-operate and work together. How do budgets, information and business cases work in practice? How functional the relationship is – is it measured in the quarterly Department of Health independent practice survey?

What is the functional responsibility of the BC consortia and how will they operate?

  • Economies of scale
  • How to work with the PCT to achieve your mutual targets
  • Agreeing the new locally agreed Direct Enhanced Service for PBC or ‘PBC Incentive Schemes’ for practices and groups
  • What role does the Commissioning Organisation have with the PCT?
  • Commissioning – active involvement or passive partner?
  • Service redesign, for example:
  • shifting work out from the secondary care provider risk assessment and simple business case outline Demand and referral management
  • Operating framework
  • Accountability agreement between PCTs and practices
  • Commissioning Plans
  • Agreement on meeting management costs Data sharing and collecting agreement
  • Conflicts of interest – PBC as a provider and a commissioner Framework for reinvestment of any savings
  • Impact of redesign on local and national priorities

The development of this toolkit has been financially supported by Procter & Gamble Pharmaceuticals (P&GP).

P&GP has not been involved in the content of the toolkit with the exception of Module 4 that includes an Osteoporosis Implementation Programme that was developed by P&GP.


 

 

Appointment to Lord Darzis Review Board

 

The National Association of Primary Care (NAPC) very much welcomes the appointment of Dr James Kingsland, its Chairman, to Lord Darzi’s review board.  The announcement was made by Ben Bradshaw, Minister of State for Health at NAPC’s Annual Conference, held on 17th and 18th of October 2007.  Mr Bradshaw, MP confirmed that the appointment of Dr Kingsland would significantly strengthen Lord Darzi’s board.

Dr Kingsland commented:  ‘I am delighted to be invited to be involved in the work of the review board on behalf of NAPC and its wider membership’.  ‘This is an important and key review for primary care’, he continued, ‘and it is critical that contributions are made by the innovative and forward thinking clinicians and managers, whom NAPC represents.  It will be important to strike the right balance in delivering solutions which address local problems, while avoiding reinvention of the wheel over and over again.’
